During the main quest “For Whom the Bell Tollsin Kingdom Come: Deliverance 2, you have to play the role of an impromptu healer. Your mission is to help Captain Thomas recover from a serious infection in order to save Hans from the execution which will happen after 12 in-game hours. But figuring out what medicine Thomas needs is a bit complicated thing. It happens in Trosky Castle where you need to find Thomas, deal with guards, and craft the right potion for him. So, here’s how to meet and give the cure medicine to Thomas in KCD 2.

After carrying the sacks of charcoal, talk to the blacksmith. He’ll give you access to his work area if you agree to help him craft a horseshoe. If you don’t want to craft it, you can steal a lockpick from a nearby shelf—it’ll come in handy ahead. From the blacksmith’s table, move towards the left and go up through the stairs. You’ll come across two doors, go through the right door. Be careful, though; this area is restricted, and guards patrol frequently.

After entering the door, again take the stairs to move upward and go inside the door on your left which will be of green color – that’s your way forward. Once you exit the hallway, you’ll face another guard who blocks your path. Use a rock to lure him away from the stairs, then sneak past him. After climbing the stairs, you’ll finally reach Thomas and his sister, Adela inside the room. Talk to her, and she will explain Thomas’ condition and ask for your help to find the cure.

Once you have found Thomas, the next step is figuring out what medicine will cure him. The game won’t spoon-feed you this information, so you’ll have to have to find the right potion. First, head back toward the blacksmith area. On the opposite side, you’ll see a door underneath a staircase. Go inside, and you’ll spot a basket with some Dried Trout next to it – pick some trout.

Once you have the trout, exit the room and climb the stairs to the upper floor. At the top, you’ll come across a guard in a big room. Instead of sneaking around or fighting him, just give him a piece of the Dried Trout you picked up earlier. Feeding him will let you pass behind him to the door without any trouble. But this area is still restricted, and not all guards will be so easily bribed. Keep an eye out and move carefully as you continue.

Go through the door inside this room and keep moving up – you will have to climb a lot of stairs. In the end, you will find the upper door on the left, go to the small hallway through it, and enter the right alchemist room where you meet a woman named Katherine. There, you will find an alchemist bench, a chest, and an alchemy book. Grab the Physician’s Journal from the bookshelf. Open your inventory, go to the “Books” section, and read the journal—it will tell you that Thomas needs Fever Tonicum and also its recipe to make the potion.

If you already have a lockpick, you can open the chest next to Catherine. Inside, you’ll find:

  • Feverfew
  • Elderberry Leaves
  • Ginger

These are the exact ingredients required to brew the Fever Tonic. If you don’t have a lockpick yet, you can get one by speaking to the woman near the blacksmith and then talking to the blacksmith himself. If you don’t have a lockpick, head upstairs to where the castle’s scribe is. Knock him out and steal his key—his chest contains the Fever Tonic.

How to Brew Fever Tonicum to Cure Captain Thomas

To brew the Fever Tonicum, you need to have the following ingredients:

  • 3 parts Feverfew
  • 2 parts Grated Ginger
  • 1 part Elderberry Leaves
  • Wine (as the base liquid)

Once you have everything, head to the alchemy bench and follow these steps:

  1. Pour wine into the cauldron.
  2. Add 3 handfuls of Feverfew into the cauldron first.
  3. Boil the mixture for two sandglass turns.
  4. Grind 1 handful of Elderberry leaves using the mortar and pestle, and add them to the cauldron.
  5. Add 2 handfuls of Grated Ginger into the mixture.
  6. Pour the potion into a Phial.

After the potion is received, return to Thomas’ room. Interacting with him triggers a cutscene where you administer the Fever Tonicum. After this, Thomas starts recovering, and you can finally move forward in the storyline and complete the main quest in Kingdom Come: Deliverance 2.
